At this time our enrollment is open to a full-time 4-day and 5-day daily schedules. A registration fee must be paid to enroll your child. We encourage all families to tour our center prior to the first day of attendance. If there is no opening we will put the child on a waiting list.
If the child is accepted, a confirmation packet will be given to the family. The completion of the following forms will be needed before the child can begin his/her first day:
- Child enrollment form
- Field trip and picture-taking permission forms
- Intake form (separate forms for children 6 weeks to 2 years and 2 years to 12 years)
- Child immunization record
- Child health examination from- must be completed by doctor and returned within 30 days after enrollment (non-applicable for school-agers 6–12 years old.
- Policy acknowledgment form signed by parent/guardian
- Emergency Card
- Emergency medical permission form
- Asthma action form
